2011-2012 BASKETBALL SIGN UPS
All Sign ups will be held at OLR GYM
Fees $125 per child and $75 for each additional sibling.
Please bring copy of resident with parent name(phone, electric, water, cable bill)
Last bring copy of Birth Certificate

on each team please arrive on time.
Mandatory Coaches Meeting
